My friends, All PNP application packages should go first to CIO Sydney NS for processing. They at CIO, will receive, check whether the application is complete or not? If application is complete then they will accept it and assign UCI number for applicant and file number for application and then send it to another local visa office for final process. It means CIO is only a processing center and check and accept the applications but local visa offices will do their final process and issue visas.

Take a look at these for more information:

Since medicals are valid for a period of 1 year, can someone recommend if its a good idea to do it before even receiving any instructions by mail (or email)?

Why do you want to waste your money for nothing? I would say just wait to hear from them. If they indeed require updated medical tests results, they will let you know.. You can send the scanned UMI tracking sheet within two days max.. So you're not actually wasting time..Hope that makes sense..
