2 VISITOR EXTENSION Form question

Hello, I hope someone can help me.

I am applying for a second Visitor Record and sections with dates in imm5708 form are confusing me a bit.
In the first Personal Details section that asks about my status (visitor) I also have to indicate Date From and To of this status. But then down below, in the Details of visit to Canada section they ask me the same date thing. Also, there's another section Coming into Canada when I have to indicate my original date of entry ( that I understand).

I would really appreciate that someone guides me a little bit through this.

1.-The first question is the dates your visitor record shows, when it starts and when it expires.
2.-Is the new dates you intend to be in Canada, from when to when.
3.- Is the date you enter to Canada by the first time.
