jz scheduled pick-up(April 21, 2010 Wed) the operator i talked to advice me to make my own checklizt(2 copies) to be included in the envelop together with other docz. zhe zaid not to use the checklizt form downloadable at the zite(canadian embazzy). any pattern or zample for checklizt plz...and plz advice how i'll organize papers. TIA

What CIC/CHC region are you applying to?

Interesting. CIC Manila has its own checklist (different regions have different requirements). So your application should be the one that's coming from the CIC Manila website. If there are additional documents that you need to attach with the application, you can write it on the the checklist you are going to send the office. Have a duplicate copy.
