REQUEST FOR SUBMISSION OF PASSPORTS

This letter is in reference to your application forpermanent residence in Canada
WHY ARE YOU RECEIVING THIS LETTER?
We are in the final stages of processing and we needyour passport(s) and the documents listed below in order to make a finaldecision. Please note, this does not guarantee that a visa will be issued.
WHAT ARE WE REQUESTING?
- Passportsfor yourself and each accompanying family member
o Each person must have their own passport and theremust be at least two blank pages in each passport
o Ordinary passports are the only type of passport intowhich a permanent resident visa may be inserted. Diplomatic, official andspecial passports are not acceptable
o If you have obtained recently a new passport, pleasesend us a copy of the bio-page by e-mail so we can update the information onyour file
o If you change your passport after your visa has beenissued, you will have to bring your old passport, visa and your new passportback to the visa office for re-issuance. There is a fee of 30,00 CDN for eachvisa re-issuance

HOW TO SUBMIT YOUR PASSPORT(S) AND DOCUMENTS?
You or someone you have authorized must submit thepassport(s) and documents in person to the visa office in Warsaw or in Ankara.

You can submit your passport(s) Monday to Thursday,from 08:30 to 11:30. You will have to come back to the Visa Office the nextbusiness day at 08:30 to get the results. We do not provide services onstatutory holidays; please visit our website www.canada.pl for the complete list.
Ankara: You orsomeone you have authorized can submit in person your passport(s) anddocuments at this address:

You can submit your passports Mondays and Wednesdays between08:30 and 10:00. If you arrive late, you will have to come back the nextavailable day for passport submission. WHAT ARE THE NEXT STEPS?
Your case will be reviewed one more time and ifeverything is in order, a final decision will be taken. You must come back tothe embassy the following day to receive the results.
If a visa is issued, it will be valid until : 05 November 2012
Please note that once your visa is issued, it cannotbe extended. It will be your responsibility to travel to Canada before thatdate.

If you authorize a third party to submit and collectyour passports, please fill in the attached letter of authorization and submitit with your passports.
If there are any changes in your family configurationor your passport information, you must notify us before presenting yourself atthe Visa Office.
