Yes there is a portion in the application package where they ask such questions such as people (Family members/friends) that were present.

I typed up mine by myself and had a Provincal Notary Public stamp and sign it, it worked perfectly in Asia. Also another good idea would be to get a Notary to stamp a copy of your birth certificate and always have extra copies of your passport, drivers and other id when you go abroad.

You are absolutely right. I was born into a pretty traditional Punjabi household while being a Canadian by birth. I have read up a lot on my rights lately and feel forever grateful to be born here. As for my mother, she surely has no voice but boy oh boy is she still creating such a big scene.
