You do. At least my husband did when applying for both SIN and Ontario health card. If you go to the Service Canada website it’ll give a list of acceptable pieces to use for address and identification.

Once you’re in Canada make sure you update your address ASAP if want to avoid any delays or confusion on their end. (Speaking from experience) It’ll save you any headaches later!
