FYI, I applied for upgrade from ECA to course-by-course on Apr 2. WES notified my by email that they had received my full credentials on Apr 16 (I didn't submit my documents again). And on Apr 26, my evaluation is completed. ECA report is still there, available for resending to IRCC if I wish.

No and yes. It really depends on each particular application status. There's no hard rule as to CIC's inner working. But I believe ghost update is always a good thing because your file is being worked on. I advise to wait 1-2 days after ghost update to see if anything happens; if not, contact...
