you don't need FBI clearance for 4 months old. When I added my 2 months old child to the application, the documents that CIC asked were her passport, medical and receipt of adding dependent fee. Gud luck

In the Annexure A form it In the Annexure A form CIO is mentioned as PO(processing office). I read somewhere in this forum about someone mentioning Ottawa as their Processing office in that form.
