Yes, I had included all the documents listed on the checklists. I had to submit signed declaration that I have canceled my voter id and ration card, I had made sure to do that process prior to submitting the application.

Hello all, I received AOR in mail today and it says I need to provide IMM5768 form. Do I resubmit the same I had provided during application with new singnature date? Or do I need fill in new details? Any help will be appreciated as my timeline is far behind at the moment.
