Hello all, I received AOR in mail today and it says I need to provide IMM5768 form. Do I resubmit the same I had provided during application with new singnature date? Or do I need fill in new details? Any help will be appreciated as my timeline is far behind at the moment.

Im First inquire with physician’s office on the status of your medical examination. Second raise a web form and include the the e medical attachment and the details of when did you got medical examination, doctor’s name and address etc. This will force officer to inquire with physician panel...
