Actually I applied for GCMS notes quoting my FSW application file #. So I got info pertaining only to FSW application. Now I ordered GCMS quoting my CEC file #. Go ahead & apply GCMS notes again using you file # , mentioned in your CEC AOR.

For Q8: First, Schedule A should not change. It should be same as your FSW application. They say "Name of the company, employer, school, facility as applicable. So you can mention your india employer & within brackets give US location. I would strongly recommend to include a separate sheet...
