You are probably right on both accounts: 1. I simply attached the bank statements to be on the safe side. 2. The fee was increased last year, after I sent my application.

try walgreens or rite-aid or any other pharmacy chain. mine were accepted alright. i took a ruler with me, and had them redo it when they did not meet the spec. i had to trim the pics myself though.

I don't know of any AINP (H1) applicant who was called for an interview. It is typically waived.
