If you can't find the label simply write your file number on the envelope you are submitting. That is what is on the label.

DMP said they will send it in 5 days. It should be there by now.

Choosing a DMP in London? Forget about The Bridge Hospital, Maidenhead. Very unreceptive and nonchalant. I cannot trust people like that. Knightsbridge Doctors on the other hand are quite excellent, just a bit pricier. Just my personal experience. It could be different for you. All the best.
