There some issues about this. I hear some time they never update the Ecase. Some people have done their medical while their Ecase showing application receive. It is all about luck I think. I am on the same boat too. Waiting to update to "in prosess".

Call CIC to make sure that your file already transfered or you have to Waite your Ecase to show up it is transfer if it is not you better send it to CPP.

No, not in canada, but out side of canada in some countries after you apply and their assessment done you have to go there to collect your visa
