I have tried uploading the same form in Chrome, Firefox, IE. negative results on all three.

I have the same problem, trying to upload IMM5708. I will try and fill in the form again from scratch and re-upload. Will report back on progress. Please everybody do the same. It will be very helpful to know when/if someone manages to upload. Thanks and good luck.

I don't know where you are located, but if you are in Toronto, check out this guy. I've used him twice. He is quick and cheap. No hassles. Hmmm, can't post links... Just google "Vinh T. Tran Law" and you'll find him.
