Difficult to say, especially, without looking at the information you provided. In your previous form, did you answer 'Yes' to any of the statutory questions? Regardless, if I were you, I would fill out the form to the best of my knowledge and submit it.

Yes, I was. However, I tried only after receiving the AoR. If my memory serves me right, I did so by using my UCI and/or Application No. Good luck with your application! :)

After submitting my documents on 30th January through the webform, just now got confirmation that the documents have been received by the responsible officer. I hope the officer reviews my documents soon and proceeds my application to the next stage! :)
