I called CIC today and the agent said my medical is extended till Oct 2021, however in the application its still showing the old date. She said it may not reflect immediately in the online profile.

So I think you got medical extension without redoing it.

As per the Case Analyst, your Eligibility is Met. Next stage will be Officer review and he will mark it Passed if everything is fine. No need to re-appear for IELTS( its locked on AOR date) Counterfoil required yes means you need visa( need to wait for PPR) to land in Canada. Hope this helps.
