PCC - if your wife has changed her only when passport was renewed and if she is having any other document proof of her earlier name with your current address, try contacting local PS and show them the mail from warsaw and ask if they could provide pcc with her former name............this could...

Congrats for the MR .............you can go to the passport office psk from where your passport is issued any time before 11.00 am. Carry the print out of the appointment along with you. Pls note that if your passport is just 1 year old along with same residential address , you will get pcc on...
