Seniors, can someone advise on this please?

Seniors, I was checking the medical forms and there is discrepancy in the email & phone number for my spouse and kid. The other details Name, DOB, File # are consistent. However my details are correct (I am the primary applicant). Do I need to worry? Please advise.

My lawyer has emailed me the medical forms this morning. Looks like CIC has sent the medical request over the weekend. Timeline: App. Filed: May 30 2013 AOR Recv: July 17 2013 Med Req : Feb 2 2013
