There is a chance that all UCI can be under the same application. In either case, you should put in the Application Number so that it becomes easier for them to identify the application.

I guess it would be wise if you send a different web form for each application. If you and your family members have different application numbers, then send one for each.

I think "Change of contact information" is more appropriate. But I cannot be sure. The safest option would be to send under both (2 webforms).

Yes, just walked in to Service Canada without any appointment and got the SIN right there. This was in early April though. I am not sure how long it would take now.
