In most the cases, for whom interview is scheduled, background check haven't started because interview is the part of eligibility review and until eligibility is not passed they don't show start of background check..

NDVO works unpredictably so anything is possible, better we should be prepared if they ask to fly at the last moment..

Yes def, probability is on higher side, they will ask you are you ready to land Canada before your medical expires. if all goes well there is higher chance of getting it on the spot. Good Luck!! Keep me posted about ur interview :)
