the form is to show the date of redoing and the very fact that u have done it and also the place it has been done not for the date!!he should have insisted on getting the form.I think he should go and get it,after all the money they get for running these simple tests the least they can do is...

!!what was the sheet like?did it have any medical terms or was it general? and did the dr do the routine tests and did he ask the same old questions? if so he should have given him a green sheet that states the location and date of the test being done.!

we have filled the form and we'll send it soon hopefully. I signed the applicant part and sent it to him via mail and he will sign the spouse part.I think this is the way u should fill it.
