One is passport, and other would be any documentation (Government issued documentation) that shows your photo, full name and date of birth. The the ideal would be deiver's license, if not then health card, which also contain all three required info. I personally do not prefer PR card.

I am also in the same boat that I did not check off any of the boxes for 2017, but check off "yes" boxes for all previous years. I hope my application doesnot get returned due to that.

Based on what you are mentioning, It looks like they review the application on whether its complete or not, right after receiving them ( I mean within two weeks), and if its complete, it stays in the queue for officers to review and send AOR and other communication, and if its deemed incomplete...

Sorry to hear that. How long did they take before returning their application, from the day they received to to the date of return.

You just need to send copy of biographical page of your passport, and copy of your driver's license (both sides). Nothing more than that, not even PR card and CoPR.
