The photos must be Canadian Passport size photos (50mm x 70 mm) They must not be more than 6 months old I was asked to get 3 photos....Would advise you to carry atleast 3 for each family member The photos must have the name of the company who shot the pic and also the date when the pic was taken

The medical request communication that I got from CIC, only has the Medical Instructions( pdf) attached to it... I dont see any filled in forms as the email content suggests.... How should I communicate this to them?
