Hi Indra, My application is similar. I emailed my IRCC office and they requested a copy of the E-Medical to update our application. We also received the IMM1707 forms even though we did medical up front. I suggest you email your IRCC office with your E-Medicals and ask them to update your file.

I think I fixed the log in issue. There was a problem with the cookies. I deleted all of them in Chrome and the login worked.

If this process is similar to parents Super Visa application, then your passport showing your mother's full name will suffice for Proof of Relationship.

The Super Visa stamp cannot be issued past the date of passport expiry. That does not automatically mean they will receive the full 10 year Super Visa though. The Officer stamping can do less than 10 years at their discretion.
