It doesn't make sense. I saw there are many cases that got AoR later but got approved within 3-4 months. The processing time at VOs could be blamed for.

My application has also reached to 7 months. I passed medical exam in May 2019. My upfront medical exam was expired. I wonder whether I have to do my remeds to submit again or no need to do it till my PR application is approved?

I think that once you submit BOWP application and get a confirmation letter from IRCC, it would be told that we (IRCC) have started processing your application.

I wonder that if what could it happen if PR application is approved before BOWP is approved? Does IRCC have to refund the BOWP application fee because once PR application is approved and PR card is granted, BOWP is not needed?

There is no way to get your OHIP renewed until you have a new work permit or get an approval confirmation for your PR application from IRCC. It is what I was told by a staff at ServiceOntario.
