Not necessary your spouse will have IELTS. but having IELTS, it will increase your score max by 20 points

Maximum of 20 points. have your ECA done, it will increase your score by max 112. + your spouse ECA will increase by max 10.

Yes, CIC took some time, discussed probably with their team mates and then answered saying you can still apply writing a letter to CIC.
