YEs it can be... withdraw that application and apply fresh. also depends on what information... the form can also be edited. however recommended would be to withdraw and apply afresh

no fix format, just that it has to be on company letter head and duly signed and stamped by authorised signatory. There are ample formats available on google images.

in-total it requires 2 years...it can be collective but not necessary continuous.

There is no equivalent stuff, you need to submit exact documents. Your employer must help you since they have offered you the job. Both the mentioned documents should be available and submitted by government. If you are inland candidate then, i would suggest speak and meet them to get these...
