Depends. i received response in 5 days from LVO first time. its been 14 days this time no reply yet. standard time is 28-30 days

Not necessarily, some people reported that they didn't get a single ghost update and received ppr directly may be getting three ghost updates is also normal. Don't lose hope !!

Firstly i pray that you get your ppr before october so you can land before winters. Secondly, my brother applied for extension in 2016 after he got visa, they cancelled his visa and granted extension for 1.3 year but he was asked to redo medicals before travelling in 2017.

did you receive any email for first ghost update like 'your application has been updated' or just your profile updated when you logged in your account ?
