I had same situation, but fortunately I had previously filled copies. I think it is better to be as truthful as possible in the application. I believe ircc can do a simple UCI search and extract all the previous apps or documents.

I have done several immigration apps myself and I have always used just one explanation letter for entire app. Just bullet point it based on the form name. You should be good...

There is an ongoing debate between what to put in family size for past 3 years in that form. I opted to include my parent in the family size for past 3 year and explicitly mentioned that I am doing so in the explanation letter. Hope that will make things clear for officer to analyze.
