I did it on 19th Jan from fortis hospital bangalore, and they are e-medical enabled. As a proof of completed medicals, i sent copy of medical form shared by NDVO with hospital stamp on it and the receipt of medical fees. Hope that helps.

I think you need to produce address proof for the residential address. It could be anyrthing like bank account with nationalized bank, or telephone connection bills on your current adress or electricity bill, or lpg connection proof at your acurrent address. If you have one, you should be fine...
