No need to sign the form..... Just fill in the particulars they have asked for..... they ask for Signatures from CC holders to have their consent for charging.....

there is a spreadsheet.....type 'global spreadsheet fsw 2014' in google... you will find it... Second and most important... try to send your application as soon as possible because CIC will entertain the applications under current guidelines only till Dec 31..... Since they have christmas...

Brother don't worry, I m a 23rd June applicant and I got my PER on 21st Oct. I called my bank (SBI) after seven days of receiving PER to just check the status of my dd (for knowledge sake only) they said its was not encashed.... so funny right... According to the CIC ppl it was encashed on 9th...
