Susan apu, This is not expert advice but it seems you have given huge go to find all these documents. I think you should furnish a cover letter where you will mention all scenario. You could get a letter from bhaiya addressing VO. Then make a full set photocopy of all these docs neatly arrange...

Bhaiya should be able to retrive his application file no, i think if u just could write up a cover letter with the details of application file no and reason for refusal, should be ok for them i feel. This is no where can be a negetive impact for your application. You will be fine inshallah...
