AFAIK, GCMS notes are generated 2-3 days before they dispatch the notes. So, it should contain most latest information.

One reason could be Spouse PCC not provided, or if provided, its not for complete period of time CIC asked for.

For your first question, once you upload all the documents, a new Submit button will appear at the bottom. By clicking that button, your documents will be submitted. Till then documents are just uploaded and not sent to CIC and if need arises, you can change uploaded documents before submitting.

I think if there's no travel history in your case, there'll be one item less VO needs to check about you, and eventually may help processing your case faster. Just a speculation.

So far I haven't seen any rejection on PSK issues PCC from India. So you should be good.

I don't think its mandatory to declare credit card liabilities, if you have required funds in your account for more than 6 months. CIC just want to make sure that you haven't borrowed money from someone for Proof of Funds. I haven't declared my home loan, personal loans and credit card...

If you have stayed in multiple addresses, I suggest you to go with PSK PCC. There's always a risk with address/time bound PCCs
