You can hire your sister as caregiver. It isn't prohibit by cic. Be truthful, don't lie. Your relativeness doesn't affect application. But your sister must meet certain requirements for LIC Program http://www.cic.gc.ca/english/work/caregiver/apply-who.asp and you should have good income for...

Go to cic.gc.ca, click "I need to check application status", do all steps and finally you get it.
