If those transcripts are from Indian university or college, then it should be ok if they are notarized.

That's what I think. I am in a similar position. I asked people who got CSQ under the same program last year and year before. According to them, they would ask documents, only if they think you are eligible. It may be wrong. Check their website maybe.

No. University has authority and they have their own official stamp. I have seen mostly, in university case they provide such things in an envelope with their university sale. I have a strong feeling that they won't ask you again for this because it's issued from a Canadian university.

This is a tricky situation. There are many people in similar situation. I wouldn't risk with Quebec immigration. In my opinion, you should give an IELTS exam and submit an updated sheet.

There is a pdf available on Quebec immigration website. It is like a sample checklist and describes what formats are required.
