    Agree with canadianwoman, proof of relationship is NOT the same as proof of cohabitation, so they're clear on what they're requesting, they are even giving you clues as to what constitutes such a proof: bank account statements showing same address for both, leases under both of your names, etc.

    That guide (IMM 5289) is for applicants applying from within Canada (referred to as "Inland"). are you applying from the US or are you applying Inland? if you're inland, then do follow IMM 5289 guide, otherwise go by IMM 3910e, which indicates to provide four photographs. Cheers.
