    I think you need to also address question 6 as IMM 5669 is also the same form as Schedule A, So what you need to do is write them and let them know it was a mistake and prior to the time you were correcting the question 4 you didn't notice the 6 ... They just need you to acknowledge the mistake...

    That's because they're doing background check and you can check from the automated service call center but your spouse has to call within Canada
