I have the same question and the answer is not clear... I want to have an open work permit, but I also would stay with the same company. Should I write my company's information? Because it asks to attach original offer of employment. And I don't want they mention it on my WP. Does anyone know...

I worked on a cruise ship, and I've been to a total of 30 countries. Most of them just for a day. I am confused how to fill up this form, once I dont have proof of all the places I've been. Does anyone had or have heard about some experience like mine? Thanks
