yes, you will need her consent. I e-mailed it to my husband and he scanned a signed copy and e-mailed it back to me. this way you don't waste time for mail. It can be done in a day.

Yes, you can go directly, it only costs $5.00. here is the link with all the info: http://www.cic.gc.ca/english/department/atip/requests-personal.asp I tried submitting it online but that link didn't work so I mailed my forms. I got the GCMS notes by e-mail ( I requested this), within 30 days.
