I did same. Attached mariage certificate and old passport scan too. Don't forget to add it in letter of explanation which I didn't. I thought they will ask for additional documents, but looking at few rejections don't take chance.

PCC validity is for 6 months for most of the countries. U will need to get PCC for all the countries you and your dependents have visited for more than 6 months. Go to CIC website to look for Approved list of Panel Doctors. U can't just go anywhere.
