THIS IS HORRIBLE... they asked for medical 3 months ago via email but we never got anything ! It was due october and now its december I have sent them an urgent email but will our file be very delayed?

IS it not the london visa office's fault for not sending us the request yet? Or is it our fault for not knowing it should have been completed by now? They specifically told us in August (in an email) that they would ask us if we need another medical done... but they never did I am so worried :(
