there is no attachment of any chart with ppr 1 these days its plainly an email with no attachment just write the required info on the blank page better to type it with the top labelled with your file# and uci # and send thats it dnt make urself confuse hope its help you

applied in march 2013 remed req 25 Sep 2014 waiting for ppr since 15 October 2014

yes application received but there are many from 2012 still waiting

its not change since day 1 its same yet

i also got remedical request around the same time and completed around mid oct waiting for ppr yet

its mandatory to take polio certificate with you at the day of appointment. because its a requested by medical center they might have told you while taking your appointment on call or may b in the email which you received from IOM center
