They are basically total insane. They do not care about family and family life. They want to follow their steps to issue PPR request letter having passport in some other department holding.

What kind of additional documents they requested. Can you list them, please. That will tell what they did not accept and result in rejection. What e-case says?

You got update after long time? :) congratulations!!. Did you get anything earlier than that after submission of application Nov 2011? Is there any change in your e-case during this time or now?

It might possible they have not received your 2nd medical. Send them prof of second medical along with 2nd medical request. Also go for third medical ASAP as you do not have other choice as you are struck. >:(
