No, the updated one is 10-2014. PM me your e-mail I will send you the PDF. I think their web site have problems. On one computer I can see it 10-2014, on the other 08-2014 ???

Fill the 3 numbers of CVV from left to right. The last one will stay empty. Fill it all in PDF and then print and sign.

It is okey that the card's CVV has 3 numbers. there are different cards with different CVV, maximum 4 numbers. There are more credit cards with only three numbers. Type of the card you should just chose - Visa, MasterCard or AmEx. depends on the type of your card (tick it)

I think it can be debit card if it has CVV code behind it. (3 or 4 numbers behind the card). All credit cards have CVV code, but not all debit cards have. That is the reason I think they mantioned credir card. If the card has no CVV code it is imposible to charge the money.
