I have not made any mistakes. I suspect I have not put 'NA' in Schedule 3 in questions where they ask something like: 'If yes, have you....' (my answer was NO) If applicable, has your spouse.... (I am single) All the other forms are correct.

A question for senior forum members. Is there any correlation in how applications for FSW-1 and FSW-3 (PhD stream) are processed? More specifically, can I estimate, for example, my PER date by looking at 2014 FSW spreadsheet? Is PhD stream processed separately?

That's right, but this form is not supposed to be filled electronically. Or, maybe, they meant a job title by "job description".

I DID provide the duties. The guide says "write a description of your occupation". I thought we are supposed to provide a short description of that job on the LEFT under "from to occupation noc". There is an empty box on that side.
