Dear, If you have mentioned all these addresses in your application from same city, you did wrong. You should write according to your CNIC because It has only space for two addresses only. Now you have to do that what they said about this.

NO, They will mention both addresses on One PCC.

Don't worry because mine is 520 days since submission of application at CIO only one docments request in July, 2015.
