I dont think you should write guarantor's name because the guarantor is for the bank proof not for this i guess IAM NOT CLEAR ABOUT IT!!

That means your documents arent notarized..! Get all your documents notarized expect medicals and then apply again and also prepare a strong SOP of your profile its like a mandatory document which VO considers mainly to review your profile All the best
