thanks reyes!bka nga hndi ako sa st. luke's kc mhigpit daw dun.i already received the medical request but minus the forms...i already sent the pictures they required me to submit bago nila ibigay yung medical forms two weeks ago but until now wla pa rin yung forms. waiting is really stressful...
