ok, i looked in my files, the form your child needs to be listed in is IMM 5406 "Additional Family Information" where you'll also list all the applicant's family members' information.

If your child holds Canadian citizenship and can obtain proof (Canadian Citizenship Certificate and/or Canadian passport) he/she does not need to be included in the application for sponsorship, but still needs to be included in the form detailing family members, I forgot what form number is.

The PR card is sent to the mailing address the principal applicant specified in the Contact Information section, question #1 of the the Applicant's Generic Application Form (IMM 0008). There is no need to sign for the mail because it's sent by regular mail not registered mail.

On a similar note, I have this question: can the sponsor go on welfare once the spouse has landed? my sponsor has been unemployed for some time and cannot collect EI so would he qualify for welfare without getting into trouble with Immigration? i'd appreciate your opinion if you've been to this...
