hi i already done with my medical last friday aug.25, bring your passport and also 1 photocopy of it. with 4 pcs. passport size picture. I went there around 2pm. I think it's better to go in the afternoon, less yung tao.you'll fill out forms, then pay 4050 pesos. and the usual physical exam...
